1. Click here to print and complete the additional services request form and be sure to complete the MMS portion
2. Click here for a direct deposit form if you would like funds automatically sent from your paycheck. Be sure to check with your Human Resource department for any additional forms your employer may need. If they do not request an additional direct deposit form, UECU will provide your employer a copy of the direct deposit form you submit to us.
3. Make sure to sign all the forms
4. Send all forms along with the required opening deposit to:
